What you need to do:

• Go to the MVD and ask for a revocation application. Ask if you are eligible for reinstatement.

OR

• Alternately, you can download a blank packet, and print it out yourself, at https://azdot.gov/docs/default-source/mvd-forms-pubs/99-0139.pdf?sfvrsn=0. It is HIGHLY recommended that you call MVD and ask if you are eligible for reinstatement, as there are NO refunds for packets submitted by me which are returned or rejected by MVD for ANY reason. So, it is in your best interest to make sure you are not wasting your time and money! If your application is rejected or returned due to a mistake on my part, I will make it right. If it is due to a mistake or your part, I can only promise to do my best to help you remedy the problem.

• At this point, you bring me the application, and together, we fill it out. You need to provide me with copies of any certificates or letters of completion for treatment, as well as a copy of the DUI screening you had done prior to beginning treatment. During our meeting, you will complete two standardized tests, as well as answering questions related to your DUI(s), current period of abstinence (1 year minimum is required), and future driving without drinking plan. This meeting takes from 30 minutes to an hour. 

• Once eapplication is complete and signed, I will submit the packet, as well as a copy of my professional license, to the MVD in Phoenix, Arizona. They will contact you directly with the results of your application, generally within 10 to 30 days. I will text or email you copies of the application, my professional license, and the receipt from Post Office.
